IntroductionPeople with serious respiratory illness experience a high symptom burden, including fatigue, which is highly prevalent and has a profound impact on daily life participation and health-related quality of life (HRQoL) [1–5]. Fatigue is reported by up to 95% of people with COPD or interstitial lung disease (ILD) and 60% of people with asthma or pulmonary hypertension [1–4].

It is likely that the burden of breathlessness in low and middle-income countries (LMICs) is much higher than has been estimated using calculations of disease burden and expected prevalence of the symptom. However, most breathlessness research has been conducted in high-income countries and may not be relevant to LMICs. To address this issue, we convened an international breathlessness and global health workshop.

Respiratory symptoms, predominantly breathlessness and cough, are widely reported by people with serious respiratory illness and adversely impact health-related quality of life (HRQoL) [2, 3]. Breathlessness may be insufficiently recognised [4] and under-treated [5].
