Despite biochemical control, 5% to 10% of patients report persistent symptoms such as mood disturbances, fatigue and "brain fog." The etiology of this is poorly understood and is believed to be multifactorial.

Low bone mineral density is an indication for parathyroid surgery, as improvement in BMD after surgery is well documented. Patient and biochemical characteristics that predict improvement in BMD, and thereby fracture risk, are not well elucidated.
