There is hardly a better example than the concept of random assignment to illustrate the tension between clinical research and clinical practice.1 Traditionally, randomized trials are designed for collective benefit optimizing the trial parameters to reach reliable conclusions for future patients. Clinical practice on the other hand is often focused on optimizing treatment for individual patients.
