One of the biggest gaps in cirrhosis management is identifying patients and getting
them into subspecialty care.
The last three decades have seen a sharp increase in the number of cirrhosis deaths in the United States, with figures reported by The Lancet Public Health in August 2024 showing an uptick of 87% between 1990 and 2019.

A 54-year-old woman is evaluated in the emergency department 3 hours after a 30-minute
episode of chest discomfort. She is currently pain free. She reports a 2-day history
of recurrent episodes of chest “fullness” while at rest. Episodes are
7/10 in severity and last 5 to 45 minutes without radiation or other associated symptoms. She also reports discomfort after meals. She has not exerted herself because of concerns
of precipitating chest discomfort.

Amid rising U.S. stroke rates, internal medicine physicians can provide a primary prevention
bulwark, encouraging behavioral changes and prescribing preventive medications, including
glucagon-like protein-1 (GLP-1) receptor agonists in eligible patients, according
to the latest evidence-based guideline.
More than 600,000 Americans annually have a first stroke, with as many as 80% of strokes considered preventable.

The assassination of UnitedHealthcare CEO Brian Thompson in December 2024 sent shock
waves across the country and through the health care industry. The subsequent public
outpouring of anger and frustration, vitriol, and negative commentary about Mr. Thompson's
employer specifically and the health insurance industry in general was unprecedented
but not surprising. It once again reminded us that there is something terribly wrong
with the business of ensuring the health of the American people.
