We are grateful for Dr. Akkar and colleagues' interest in and succinct commentary on our study examining the association between early electrolyte monitoring and hospitalizations with hyponatremia among older adults starting antidepressants. Our sample's median serum sodium concentration was 132 mmol/L at the time of hospital admission, in keeping with relatively mild hyponatremia.

Key points In a population-based, retrospective cohort study of 420,085 older adults starting selective serotonin reuptake inhibitors (SSRIs) or selective norepinephrine reuptake inhibitors (SNRIs), we found a two-fold increased risk of hospital visit with hyponatremia among those who had their serum electrolytes tested in the week following SSRI/SNRI initiation, likely due to residual confounding. Hospital visits with hyponatremia after starting SSRI/SNRI were uncommon.

KEY POINTSRecreational use of nitrous oxide is a growing problem in many jurisdictions, including Canada. Although isolated, short-term use rarely leads to serious complications, chronic use can cause neurotoxicity that is often not fully reversible; cervical myelopathy, peripheral neuropathy and encephalopathy have been described. The pathophysiology of nitrous oxide toxicity results from functional vitamin B12 deficiency.

Dear Editor,We thank Dr. Francis for her comments. Our article explicitly discusses breastfeeding, and we acknowledge that we lack data on breastfeeding status. The discrepancy between the numbers in the text and those in Figure 2 are explained by censoring. We censored mother-infant pairs if a mother who initially did not fill an opioid prescription subsequently filled one in the 30 days of follow-up.
