We are grateful for Dr. Akkar and colleagues' interest in and succinct commentary on our study examining the association between early electrolyte monitoring and hospitalizations with hyponatremia among older adults starting antidepressants. Key points In a population-based, retrospective cohort study of 420,085 older adults starting selective serotonin reuptake inhibitors (SSRIs) or selective norepinephrine reuptake inhibitors (SNRIs), we found a two-fold increased risk of hospital visit with hyponatremia among those who had their serum electrolytes tested in the week following SSRI/SNRI initiation, likely due to residual confounding. Hospital visits with hyponatremia after starting SSRI/SNRI were uncommon.
