I’ve taken a little writing break as I turned off news to preserve my mental health after the election. As 2024 limps to a close, I look forward to a fresh start in 2025 with a few of my own crystal ball predictions of what’s to come in American health care. Accessing primary care will continue to be a challenge, not just in rural communities but in big cities too.

Having spent two years in Oxford as a Marshall Scholar in the mid-1970s, I came back to medical school believing in a national health service—i.e., Medicare for all. But I learned firsthand that a monopoly can provide access without ensuring quality. When quality isn’t the priority, there’s no hope for lowering costs or alleviating suffering. Medicare is already the single payer for dialysis and has failed patients miserably.

And—in an extremely rare event—one of them was just kicked off a voting machines case. Several Trump and GOP attorneys have either pleaded guilty, been indicted or faced bar association discipline for their 2020 actions.

The focus of former President Donald Trump’s 2024 post-election strategy may be to interfere with certification of the election results in the seven or eight battleground states. In 2020, Trump engaged in a multistate fake elector coordinated effort to file false voting certificates, tampering with voting machines in at least three states to figure out how to manipulate the voting machines and the Jan. 6 riot to prevent the counting of the electoral college votes.
