I’ve taken a little writing break as I turned off news to preserve my mental health after the election. As 2024 limps to a close, I look forward to a fresh start in 2025 with a few of my own crystal ball predictions of what’s to come in American health care. Accessing primary care will continue to be a challenge, not just in rural communities but in big cities too.

Having spent two years in Oxford as a Marshall Scholar in the mid-1970s, I came back to medical school believing in a national health service—i.e., Medicare for all. But I learned firsthand that a monopoly can provide access without ensuring quality. When quality isn’t the priority, there’s no hope for lowering costs or alleviating suffering. Medicare is already the single payer for dialysis and has failed patients miserably.

When demand exceeds supply, a resource is limited. Allocating those resources “involves a tension between efficiency and equity.” The market has several ways to allot those limited resources, beginning with cost. For example, frequent flyer miles, a form of resource allocation, may not seem controversial, except to those getting on last, where there is no room for their carry-on. Health care is a limited resource for many reasons, and equity raises more significant ethical concerns.

Since January 1, 2023, the Transparency in Coverage Rule required health insurance providers and health insurers to provide price information to participants in a publicly accessible place. This means consumers should, in theory, be able to go to any payer’s website and see what different doctors, hospitals, and health systems are charging for the same services.

In September 2023, President Biden announced health care provider training, allocating over $8 million through 18 awards to train primary care medical students, physician assistant students, and medical residents in providing culturally and linguistically appropriate care for individuals with limited English proficiency, as well as those with physical or intellectual and developmental disabilities.
