This study is a two-arm, multicentred, randomised controlled trial. Outcomes will be measured at baseline, three, and six-month follow-up time points. Figure 2 represents the study design and participant flow. The study flow chart depicts the two pathways; the intervention and usual care group. The usual care group will be screened for malnutrition at their healthcare services and will be referred to a dietitian when clinically indicated.
