At PagerDuty, I led the effort to overhaul our notifications platform. It pains me to admit how over budget and behind schedule the project was, but I’d make the same decision in a heartbeat even if the true cost had been better understood upfront.

Software development teams (whether they directly follow agile principles or not) typically attempt to improve their processes over time. For the agile-oriented folks, this is done in retrospectives.
