Our largest fundraiser of the year was Thursday, September 12, at the John Miller Center in Roberts Park. This is always an encouraging, well attended event of over 200 like-minded individuals in our community. Thank you to our caterers, C & T Catering, Ole Rooste, and Your Way Catering, for providing the delicious food! Our theme this year was “Hope in the Night“. We chose this theme because there have been several opportunities for night faith this year. We all prefer full sun faith.

Clinical questionWhat are the risks and benefits of using topical corticosteroids for atopic dermatitis in children and adults? Bottom lineWhile evidence is limited, topical steroids are effective against atopic dermatitis and efficacy likely increases with potency. Use once daily seems as effective as twice daily. Treatment 2 days per week of areas with frequent recurrent flares will help about 60% avoid a flare versus about 30% using a placebo over 16 weeks.

First of all, you are not alone. Patients miss doses for a variety of reasons—everything from a change in routine, such as travel or an unscheduled appointment or event, to a lapse in memory. But skipping a dose can render medication less effective or cause symptoms to reappear. For example, missing a dose of blood pressure medication may require additional blood pressure monitoring throughout the day. And forgetting to take blood thinners may raise the risk of having a blood clot or stroke.

Clinical questionDo cranberry products prevent recurrent urinary tract infections (UTIs)? Bottom lineEvidence is of low quality because of potential publication bias, small studies, and unblinding. If biases are disregarded, cranberry products might reduce the proportion of women with recurrent UTIs to 18% vs 24% with placebo over 1 year or less. Results are inconsistent between populations (eg, children with recurrent UTIs may benefit but not institutionalized elderly patients or pregnant women).

Clinical questionAre orexin antagonists safe and effective for treatment of primary insomnia? Bottom lineOrexin antagonists help people fall asleep about 9 minutes faster and increase total sleep time by about 19 minutes versus placebo over the course of 1 to 3 months. About 8% of those taking orexin antagonists will experience next-day somnolence compared with 2% taking placebo. EvidenceSeven systematic reviews of RCTs were found1-7; we report mainly on the most recent and comprehensive review.

One of the most common presentations in primary care is the uncomplicated urinary tract infection (UTI), characterized by patient reports of increased frequency, dysuria, urgency, incomplete emptying, or hematuria. It is traditionally ascribed to bacterial overgrowth restricted to the bladder. Another common situation is that of abnormal urinalysis or culture findings from someone (usually 60 or older) who has nonspecific symptoms but no clear urinary symptoms.
