A man in his 20s presented to the dermatology department with a monthlong history of gradually progressing papules and nodules on the face, accompanied by fever, fatigue, and weight loss. On physical examination, multiple pink papules and umbilicated nodules were found on the face (Figure 1A). He had a 5-year history of HIV infection but self-discontinued his antiretroviral treatment due to treatment-related gastrointestinal and neurologic reactions 4 years prior.

1 IntroductionPrurigo nodularis (PN) is a relatively uncommon dermatosis characterized by recalcitrant chronic pruritus and keratotic nodules (1). It predominantly affects middle-aged and older individuals and Africans (2). Multiple discrete nodules are often symmetrically distributed in scratchable areas, such as the extensor surfaces of the limbs and trunk, with fewer lesions on the harder-to-reach mid-upper back, creating the classical butterfly sign (3).

1 IntroductionUrticaria is a frequent allergic skin disease that occurs in the spring (1). The prevalence of urticaria in adults is about 15-23%, with the cause being undetermined in 70% of these patients (2). Chronic spontaneous urticaria (CSU) is defined as the unprovoked appearance of wheals, erythema, and severe skin pruritus, lasting less than 24 hours (as opposed to urticarial vasculitis) and relapsing for more than 6 weeks (3, 4).
