This issue of The Journal of Pharmacology and Experimental Therapeutics contains a comprehensive paper evaluating a novel therapy for coronary vasomotor disorders (Tawa et al., 2024). There is a desperate need to develop new therapies for these diagnostically elusive disorders that are often difficult to treat. Basic science studies play a fundamental role in identifying these therapies but are challenging to translate into a clinical context.

This special focus issue on coronary artery spasm is dedicated to the memory of Prof Attilio Maseri, a cardiology giant, who sadly died in 2021 but not without leaving an indelible legacy. Born in Udine, Italy, Prof Maseri trained in cardiology and nuclear medicine. He held fellowship positions at Columbia University and Johns Hopkins University in the US.

Abstract Coronary spasm (CS), which may occur at the epicardial (focal or diffuse spasm) and/or microvascular (microvascular spasm) level, is a well-established cause of myocardial ischaemia, in particular in patients with anginal chest pain despite unobstructed coronary arteries. The diagnosis of CS can be confirmed during coronary angiography by an additional provocation test with vasoactive substances such as acetylcholine.
