Permanent regulatory improvements to the methadone treatment system have begun, and more are urgently needed. Overdose rates are higher than ever before due to fentanyl; and opioid use disorder (OUD) with fentanyl is a condition for which methadone may be a particularly effective treatment. The US Opioid Overdose Epidemic In The Post-COVID-19 EraOpioid overdoses remain high and stable since 2021, driven by high-potency synthetic opioids (that is, fentanyl).

More than twenty years have passed since the Institute of Medicine (now the National Academy of Medicine) published its 2003 landmark report, Unequal Treatment: Confronting Racial and Ethnic Disparities in Health Care. The central conclusion of the report was that racial and ethnic disparities were a hallmark of the US health care system and that unequal treatment persisted even when controlling for health insurance coverage.
