MethodsWe will conduct a scoping review to examine the transitional care literature for older adults living with disabilities to better understand data collection methods applicable to older adults with age-related disabilities. A scoping review methodology was selected to allow us to explore the broad research topic and obtain knowledge from across study designs [44, 45].
