Although individuals with systemic lupus erythematosus (SLE) today are much more likely than ever before to live normal lifespans, many still accumulate substantial organ damage over their lifetime. Damage can accumulate both from the effects of the disease itself and from drug toxicity, particularly from glucocorticoids.

Dear Editor,The 2023 update of the EULAR recommendations for the management of systemic lupus erythematosus (SLE) recommend hydroxychloroquine (HCQ) for all patients with SLE at a target dose of 5 mg/kg real body weight (RBW)/day but also suggest individualizing the regimen in accordance with the risk of flares or retinal toxicity.

DiscussionSPOCS is a prospective, observational study designed to analyse clinical outcomes over time of a large, real-world cohort of patients with moderate-to-severe SLE. Overall, this study identified substantial clinical burden in a broadly representative group of patients with moderate-to-severe SLE in a real-world setting, including long disease duration and presence of organ damage in more than half of patients.
