A 49-year-old male patient with an extravascular implantable cardioverter-defibrillator (EV-ICD) (Aurora system, Medtronic) experienced major discomfort, mainly psychological, with the device. •To understand that the extraction of the substernal lead of the EV-ICD should be performed using a dedicated workflow in a center experienced in mechanical tool use and with surgical backup. •To note that more clinical data will be needed on extraction of older leads beyond 5 or 10 years after implantation.
