A 49-year-old male patient with an extravascular implantable cardioverter-defibrillator (EV-ICD) (Aurora system, Medtronic) experienced major discomfort, mainly psychological, with the device. •To understand that the extraction of the substernal lead of the EV-ICD should be performed using a dedicated workflow in a center experienced in mechanical tool use and with surgical backup. •To note that more clinical data will be needed on extraction of older leads beyond 5 or 10 years after implantation.

1 INTRODUCTION Implantable cardioverter-defibrillator (ICD) therapy is associated with a mortality benefit for patients at risk for sudden cardiac death.1-4 Understanding the costs of ICD therapy involves not only the costs of initial implantation but also the ongoing costs related to standard device follow-up, generator changes for battery depletion, and the management of device-related complications over the life of the patient. Innovations in ICD technology can impact these costs.
