The Occupational Safety and Health Administration has completed its investigation into the May 8 death of employee Kathleen Galloway-Menke is complete. OSHA cites three violations, two being serious. The youth group home serves some of the state's most troubled teens and is a 501(c)3 nonprofit. Galloway-Menke suffered a head injury after she was pushed by a teen at the home, the report cites. She died days later at a local hospital.

Several bills still alive at the statehouse are tied to the state's high rate of new incidences of cancer. The issue even came up in Gov. Kim Reynolds' Condition of the State address. "Every case of cancer is a tragedy," she said in January. "And I'm concerned by the data showing that these tragedies disproportionately affect Iowans."Reynolds called for $1 million to start researching the reasons why cancer rates are rising in Iowa. Lisa Bistline of Runnells would like to know.

During the steamy last days of August, we came to know Phill, the wandering water buffalo. The animal took a four-day summer trip through the Polk County town of Pleasant Hill (hence, his name), checking out backyards, a bank parking lot and other locales. Phill became a bit of a local celebrity, and now he's living the good life at the Iowa Farm Sanctuary after seemingly facing death twice.
