1. IntroductionAdvances in cancer care and treatment among children with cancer have markedly improved survival. Over 85% of children diagnosed with cancer will survive five years, with an estimated 500,000 survivors of childhood cancer living in the United States today [1]. However, this success is not without substantial cost for survivors. Treatment-related consequences and late effects are prevalent [2,3,4,5].
