Article Highlights •A new tibial anatomic morphology scoring system based on the SYNTAX Score is presented. •Lesion calcification, vessel diameter<3.0 mm, vessel length>300mm, and pedal runoff define morphology. •Three risk categories: low (0 or 1 factor), moderate (2 factors), and high (3 - 4 factors). •Increasing risk categories is associated with poorer short and long-term outcomes. •Defining tibial morphology allows a better dichotomization of outcomes.
