Obesity is a major global health concern that strains available healthcare resources. The prevalence of obesity has doubled since 1980 in over 70 countries1 and is known to increase the risk of colorectal cancer2–4. As a result, the demand for curative surgical interventions in overweight and obese individuals is expected to grow.

Radiotherapy (RT) has been shown to decrease the rate of local recurrence (LR) in rectal cancer1,2. However, with the advent of the total mesorectal excision (TME) technique on one side and the widespread use of neoadjuvant chemotherapy on the other side, the necessity of RT for local disease control has been questioned.
