A pilot who died after his De Havilland Chipmunk crashed at Jandakot Airport last April was likely distracted by a flapping engine cover. An ATSB report said the engine cover on the plane had flapped up and down shortly after take-off. A part of the plane was also found to have been fitted with non-conforming rivets, compromising its crash-worthiness.

The organisation, a lobby group for WA's agricultural sector, appointed RSM Australia partners Jerome Mohen and Greg Dudley as administrators on Thursday afternoon.

The Perth couple who survived a plane crash off Rottnest Island last week, which killed two tourists as well as the pilot, have issued a statement to say thank you for the prompt response from rescuers and medical personnel. Jeremy and Patricia Connor were aboard the plane, operated by Swan River Seaplanes, which crashed into the ocean in Thomson Bay last Tuesday afternoon. On Wednesday, police divers recovered the bodies of two tourist passengers and the aircraft's pilot James Wong.

WA authorities have warned there is "still a bit of work ahead" as a massive bushfire that ripped through almost 80,000 hectares of land continues to burn. The blaze, which began on Monday morning, threatened the coastal towns of Cervantes, Wedge Island and Grey, prompting a massive response from the Department of Fire and Emergency Services (DFES), volunteer firefighters and the Department of Biodiversity, Conservation and Attractions (DBCA).

West Coast Eagles defender Tom Barrass has told the club he wants to be traded to Hawthorn. Barrass, who will turn 29 in October, reportedly met with Hawthorn coach Sam Mitchell — his former teammate and assistant coach at West Coast — last month. Barrass has not played for the Eagles since that time, with concussion and then a hamstring injury keeping him out of the club's final four matches of the season.

West Coast Eagles midfielder Andrew Gaff has announced his retirement from the AFL. Gaff, who turned 32 in June, has played just three AFL matches this season. He was dropped after a disappointing outing against Port Adelaide in round one, but was recalled to the senior side for their matches against St Kilda and North Melbourne. The club says he will play against North Melbourne in Hobart this weekend.

AFL chief executive Andrew Dillon has defended the league's tribunal system, as news emerged both Brisbane and GWS would appeal suspensions handed to their players over tackles on the weekend. The role of the tackle in the AFL has been thrown into the spotlight over the past few days, with players receiving lengthy bans for footballing actions that would not have raised an eyebrow just a few years ago, because the players they tackled suffered concussion.

In the end, it was inevitable. Adam Simpson had to go as coach of West Coast. Some improved performances late last year, and three wins this season after the arrival of young draftee Harley Reid, had Eagles supporters thinking they had turned a corner. But since the club's bye they have looked shot. Injury and illness certainly played their part in 2022 and 2023, but it was the nature of the losses and the length of the club's malaise which has shifted the dial towards a new start.
