MethodsEPISCAN II is a national, multicentre, cross-sectional, population-based epidemiological study, whose protocol, fieldwork and methods have been previously described.15 16 Briefly, the study was conducted at 20 teaching hospitals throughout Spain from April 2017 to February 2019. Patients and public were not involved in the development of the research question, design or recruitment; no patient advisors were required, and data were analysed anonymously.
