As a card-carrying member of the Coalition of Type A Persons, I certainly understand the need for certainty and the desire to plan. I am constantly trying to find a more efficient way to organize my day and create space for new organizational structures. In most ways, the act of working in the ED fits this desire and this need very well. Patients are pieces to move through a system and as you figure out the best ways to move them through, that system you gradually become more efficient.

He was intubated and deeply sedated, hypoxemic from multifocal pneumonia, and on two vasopressors for his septic shock. Another gentleman was cachectic, and barely able to walk to the bathroom on his own due to extensive left lower extremity pyomyostitis. Yet another patient experienced acute respiratory failure from Legionella pneumonia, and required 8 liters of oxygen. There was also the frail young man, just a few years older than I, on inotropic support for decompensated heart failure.

Leaving all I knew behind and immigrating to a homogenous neighborhood as a 7-year-old was a difficult transition. I couldn’t understand why no Disney channel heroine looked like me; why I felt so abnormal. Years and assimilation pass: I am 21 years old and one of the refugee women I work with asks to speak to me in private. She is embarrassed to tell that she hasn’t been able to attend our meetings on navigating the American health care system because she and her neighbors lack transportation.

The best way of describing a student rotating with me is the feeling I have when I plan my long runs — I really don’t look forward to it, but once I start, I enjoy the feeling, and when it’s done, I am glad I did it and have a sense of accomplishment. I wasn’t particularly looking forward to another student rotation when I walked into the cath lab. “Anyone seen my student?” I asked.

Patients have issues over the weekend and reach the physician on call, or perhaps a nurse in the ER, over the phone. The reasons are myriad. They have pain, nausea, a rash that is getting larger, swelling in the operative leg. They want to know: Is this normal? Will it get worse? Could it be something serious? But ultimately, what they all want to know is whether they should go to the ER.

As a child, I had a vivid image of myself as a physician wearing the smartly pressed long white coat and holding a brilliantly shining stethoscope as I listened intently to the hearts of my patients. At just eight years old, I would ravenously devour the high gloss, vividly colored pages of the human anatomy encyclopedias my mother gave me.
