Are Physicians Really Part of the Middle Class Now? Whether we like it or not, physicians are merging into the middle class. I don’t mean that in a strictly financial sense. While the trend for physician income is depressingly down, the middle class (based on the Pew Research Center definition) makes two-thirds to double the U.S. median household income or roughly $50,000–$150,000 a year. Most physicians are doing better than that.

When I was a child, I used to snoop in the pantry of my grandparents, where I would find bags upon bags of mystery ingredients. As an adult, I now know the ingredients to be goji berries and white fungus. My grandmother was an endocrinologist and my grandfather was an infectious disease physician who practiced what we view as conventional Western medicine. But so integral in daily living in China, where my grandparents were from, was the almost inherent knowledge of traditional Chinese medicine.

A two-hour drive. A subway ride. Two flights. Land at 1 a.m. Crash for a few hours. Another flight. A death-defying hour-long taxi ride winding through the mountains at alarming speed, passing trucks on blind curves, traversing fault lines that made the road shift and heave. Why AI Won’t Replace Physicians Any Time Soon (or, The Value of Being There in Person)There’s a patient who comes to see me every week. Every time we part ways, I say, “OK Susan! See you in a month!” But inevitably, she’ll call on a morning when she knows I’m in the office and my staff will fit her in. She likes to tell me about what she did with her granddaughter that week. She doesn’t have anything terribly wrong with her. She just wants some reassurance.

Unless you’re from a very specific geographic region, the name New York probably conjures images of skyscrapers, subways, restaurants, and a city that never sleeps. Also traffic, walls of odd smells, and rats scurrying around piles of sidewalk garbage. All these pictures are accurate, but forget that New York isn’t just New York City, but a whole state, which features trees, beaches, and farms.

In my early 20s, I lived in New York City and was a frequent patron at many (too many) bars where smoking was still allowed. When indoor smoking was banned, I was strongly against it on the grounds that people should have the freedom to choose what to do with their bodies. Then, on a trip to Europe before starting medical school, I was trapped in an unventilated train car with a central European soccer team while traveling from Nice to Switzerland.

As a card-carrying member of the Coalition of Type A Persons, I certainly understand the need for certainty and the desire to plan. I am constantly trying to find a more efficient way to organize my day and create space for new organizational structures. In most ways, the act of working in the ED fits this desire and this need very well. Patients are pieces to move through a system and as you figure out the best ways to move them through, that system you gradually become more efficient.

He was intubated and deeply sedated, hypoxemic from multifocal pneumonia, and on two vasopressors for his septic shock. Another gentleman was cachectic, and barely able to walk to the bathroom on his own due to extensive left lower extremity pyomyostitis. Yet another patient experienced acute respiratory failure from Legionella pneumonia, and required 8 liters of oxygen. There was also the frail young man, just a few years older than I, on inotropic support for decompensated heart failure.

Leaving all I knew behind and immigrating to a homogenous neighborhood as a 7-year-old was a difficult transition. I couldn’t understand why no Disney channel heroine looked like me; why I felt so abnormal. Years and assimilation pass: I am 21 years old and one of the refugee women I work with asks to speak to me in private. She is embarrassed to tell that she hasn’t been able to attend our meetings on navigating the American health care system because she and her neighbors lack transportation.
