I can’t say that I wasn’t warned. It was 2017 and I was the CEO of a 6-physician pediatric practice. We were doing fine, but quality incentive dollars formed at least half of our annual revenue. We were behind on our latest Patient Centered Medical Home (PCMH) recertification and at risk of not making payroll. I was sitting in my own doctor’s office, following up on stress-related health issues, when I saw a parent who worked there as a medical assistant.

“But what if I can’t find anyone to pick me up?” My silence must have hung heavily over the phone as I tried to think of solutions to this impossible question. The patient on the other end of the line was a gregarious middle-aged woman whose Cologuard had recently returned positive, and she needed to be scheduled for a colonoscopy. Unfortunately, she couldn’t think of anyone who would be able to pick her up from her procedure, or provide aftercare in the 24 hours post-anesthesia.

Unless you’re from a very specific geographic region, the name New York probably conjures images of skyscrapers, subways, restaurants, and a city that never sleeps. Also traffic, walls of odd smells, and rats scurrying around piles of sidewalk garbage. All these pictures are accurate, but forget that New York isn’t just New York City, but a whole state, which features trees, beaches, and farms.

“She loves eating 'girl dinner' every night," my patient's mom proclaimed while rolling her eyes. I had asked if her 13-year-old was a picky eater at her annual well check. I was confused by what the mom meant until I remembered a recent TikTok that I had seen where a group of girls were eating nuts, popcorn, fruit, and chocolate for dinner with the words girl dinner in a sparkly font written across the video.
