Editorial independence safeguards the rights of authors and readers to freely speak and be heard (Brakoulias and Malhi, 2024; Hoey, 2008; Looi et al., 2024b). This underpins the key goals of medical journals to strive for academic excellence and progress the science of care (Malhi and Brakoulias, 2022). It is enacted by editors having freedom to make decisions about the scientific publication record (Bhui et al., 2025).

Academic psychiatry and publishing have undergone fundamental changes in the last decade. Both face challenges, from within the field and outside of it. Internal challenges include debates over the scope of mental illness, a shrinking number of clinician-scientists and an excessive focus on biological research with limited relevance to clinical practice (Kisely, 2015; Kleinman, 2012).

INTRODUCTION Smoking prevalence is substantially higher in people with psychosis compared with the general population (65% vs. 11.6%; AIHW, 2019; de Leon & Diaz, 2005) contributing to increased risk of cardiovascular and respiratory diseases and early mortality (Cotton et al., 2022; Dickerson et al., 2014). Explanations for the high prevalence of smoking have included self-medication, causal relationship and shared liability theories (Sharma et al., 2016).
