1 INTRODUCTION It is well established that breastfeeding and human milk protect maternal and infant health (Victora et al., 2016). When babies cannot receive their own mother's milk, donor human milk (DHM) may protect premature infant health and development, reducing the risk of necrotizing enterocolitis and other complications such as bronchopulmonary dysplasia (Quigley et al., 2019; Villamor-Martínez et al., 2018).

1 INTRODUCTION The complex link between breastfeeding and maternal mental health is well-established (Alimi et al., 2022; Brown, 2018; Da Silva Tanganhito et al., 2020). For mothers who want to breastfeed, meeting their personal breastfeeding goals can support wellbeing and increase parenting satisfaction (Avilla et al., 2020; Shepherd et al., 2017). This can be particularly impactful when an infant is unwell or has been born prematurely (Flacking et al., 2016; Hookway et al., 2023).

Abstract Background Breastfeeding is an important public health priority and may be particularly beneficial for medically complex infants and children. However, childhood illness and disability are associated with increased challenges and lower breastfeeding rates. The Baby Friendly Initiative has been shown to increase initiation of breastfeeding and improve health professional skills although as yet the standards have not been adopted in paediatrics.
