The burden of organ damage and flares in patients with systemic lupus erythematosus (SLE) is substantial. While flares are known to increase the risk of organ damage, prospectively collected longitudinal data quantifying the independent association between flares and organ damage are limited.

DiscussionAttainment of low disease activity in SLE is included in the updated EULAR recommendations for SLE as a treatment target.22 LLDAS is the most widely accepted and validated23–27 low disease activity end point for SLE. According to the definition of LLDAS, major organ system activity such as renal, central nervous system, cardiopulmonary, vasculitis, fever, haemolytic anaemia or gastrointestinal activity is excluded.

Summary Validation of protective associations of the lupus low disease activity state (LLDAS) against flare, irreversible damage, health-related quality of life, and mortality has enabled the adoption of treat-to-target strategies in patients with systemic lupus erythematosus (SLE). Previous validation studies were of short duration, limiting the ability to detect longer term signals in flare rate and irreversible damage.
