Severe ABI refers to a sudden event that results in brain damage and reduced perfusion leading to reduced alertness. ABI is heterogeneous and covers different subtypes, notably traumatic brain injury (TBI), subarachnoid hemorrhage (SAH), intracranial bleeding and hypoxic ischemic brain injury.

As with any intervention, the ultra-lung-protective ventilation strategy does not come without risks. Indeed, the resultant decrease in mean airway pressure could cause lung derecruitment, atelectrauma, and biotrauma. Lung collapse and overdistension may also occur simultaneously in severely injured lungs.
