It is no longer a matter of obeying a principle of order (excluding disorder), of clarity (excluding obscurity), of distinction (excluding adherences, participations and communications), of disjunction (excluding subject, antinomy, complexity), that is to say a principle which ties science to logical simplification. On the contrary, starting from a principle of complexity, it is a matter of linking what was disjointed.

Yet it is essential to study them using approaches adapted to their complexity, the plurality of their mechanisms, and the diversity of local actors and contexts. AbstractLa santé publique s'oriente de plus en plus vers l'étude des structures oppressives (telles que le racisme, le sexisme, ou le validisme) et de leur influence sur le marché de l’emploi, les systèmes éducatifs et judiciaires, et l’accès aux systèmes de santé de qualité. Ce commentaire vise à proposer une réflexion sur la manière dont ces structures influencent également la façon de faire de la santé publique.

Les systèmes de santé sont avant tout des systèmes sociaux. La manière dont ils fonctionnent aujourd’hui s’inscrit dans une histoire politique nationale mais aussi dans la diffusion d’idées et de croyances à l’échelle internationale concernant la manière dont ils devraient être organisés. Or, depuis plusieurs décennies, en de nombreux lieux de la planète, ce sont les idées néolibérales qui ont le vent en poupe et qui ont inspiré les réformes des systèmes de santé.
