AbstractMediation analysis helps uncover how exposures impact outcomes through intermediate variables. Traditional mean-based total mediation effect measures can suffer from the cancellation of opposite component-wise effects and existing methods often lack the power to capture weak effects in high-dimensional mediators. Additionally, most existing work has focused on continuous outcomes, with limited attention to binary outcomes, particularly in case-control studies.

1 Introduction Atherosclerosis is a critical pathological condition characterized by the progressive accumulation of lipids, cholesterol, and other metabolic by-products within the arterial wall, ultimately leading to the formation of atherosclerotic plaques.[1, 2] The narrowing or obstruction of the arterial lumen caused by these plaques impedes blood flow, thereby precipitating tissue ischemia.[3] Atherosclerosis represents a significant challenge to healthcare systems worldwide, given its...

AbstractMeta-analysis is used to aggregate the effects of interest across multiple studies, while its methodology is largely underexplored in mediation analysis, particularly in estimating the total mediation effect of high-dimensional omics mediators.
