OMAHA, Neb. (WOWT) - The 3-year-old girl who died from an accidental gunshot wound over the weekend had found her fatherβs gun under a couch cushion, according to the affidavit filed Monday in Douglas County Court. The girlβs family β three siblings and both parents β were at the home when it happened. Omaha Police responded at 1:46 p.m. Saturday and pronounced the 3-year-old dead at the scene, the document states. Her father, Phuc Le, was subsequently arrested and booked into jail.

HAMBURG, Iowa (WOWT) - Two Shenandoah parents have been charged after an investigation found that two children were being severely neglected. Dustin Carl Lee Perry, 26, and Lindsey Barbara Marie Hamilton, 26, both of Shenandoah, have been charged with neglect or abandonment of a dependent, a Class C Felony in Iowa; and child endangerment, an aggravated misdemeanor. They are being held on $10,000 cash bonds.
